COVID-19 Staff Vaccine/Testing Form


The New York State Health Commissioner has issued a weekly COVID-19 testing requirement for all school employees. Staff who are fully vaccinated can be exempt from the weekly testing requirement. Please complete this form to submit proof of vaccination or to indicate that you will be submitting weekly proof of COVID-19 testing, once required.

All information collected will remain confidential. Employees who need to submit weekly testing will receive further instructions.
